*Created monthly group schedules

*Managed incident reports and submitted them to the Ahcccs QM portal

*Worked closely with Therapist, Case Manager, leadership and outside agencies to provide care coordination and promote positive client outcome

* Transported residents to outside doctors appointments, meeting and outing as needed
