The 2MATCH Advocate provides support for the 2MATCH Accountable Healthcare Community (AHC) Program which seeks to address health-related social needs identified by Medicare and Medicaid beneficiaries (beneficiaries) who are receiving or have received care in the 2MATCH Clinical Delivery Sites located in the Phoenix metropolitan area. The 2MATCH Advocate will work primarily in St. Joseph’s Hospital and Medical Center and participating outpatient clinics located throughout the designated service area to support the needs of the project, support clinical staff and community partners in the field, and provide assistance by phone or in-person to high-risk beneficiaries by connecting them to community services to meet their identified health-related social needs. The 2MATCH Advocates will report to the 2MATCH Advocate Supervisor and will work in collaboration with the Dignity Health 2MATCH Program Staff to meet the outcomes of the project. They will be knowledgeable of the 2MATCH Program screening and navigation guidelines as well as with Healthify, the non-CMS Data System that will be utilized in the program.
