Field Supervisor

• Conduct routine in-home supervisory visits with clients and caregivers
• Observe caregivers during service delivery
• Ensure caregivers follow the client’s plan of care
• Identify care concerns, safety risks, or training needs
• Act as the primary liaison between clients, caregivers, and office staff/management to resolve issues and improve client satisfaction.

Caregiver Oversight, Training and Support

• Provide coaching, guidance, and feedback to caregiver
• Train caregivers in home health aide skills, provide in-service education in the field, and support staff with complex cases.
• Conduct Warm Welcome visits to introduce new caregivers to clients
• Assist with scheduling coverage for clients as needed.
• Support new caregiver field orientation and shadow visits
• Address professionalism, attendance, and performance concerns
• Report issues to office leadership as needed

Client Support

• Serve as the agency’s on-site representative during visits
• Address client concerns related to caregiver performance or care quality
• Report client feedback, complaints, or service issues promptly

Crisis Management

• Respond to, address, and resolve client and caregiver issues or emergencies in real time.

Documentation & Compliance

• Complete Supervisory Visit Documentation/Warm Welcome Forms/ New Client Intake Paperwork accurately and on time
• Ensure compliance with agency policies and state regulations
• Report incidents, safety concerns, or suspected neglect/abuse immediately

Intake Coordinator/Client Assessments & Care Plans

• Conduct new client assessments/intakes, develop customized care plans including clients ADL’s, update client care plans based on changing client needs.
• Communicate visit outcomes to coordinators, scheduling, and management
• Support retention and quality improvement initiatives

Household & Companionship (IADLs)

• Companionship: Engaging in conversation, hobbies, games, and social activities to reduce isolation.

Health & Safety Monitoring

• Medication Reminders: Reminding clients to take medication (not administering), reading labels, and assisting with pre-filled dispensers.
• Safety Monitoring: Ensuring a safe environment for client, such as preventing falls or identifying safety hazards.
• Observation: Reporting changes in clients physical or mental health to family members and health care professionals.
• Transportation: Coordinated and provided transportation for medical appointments, social events, and essential errands.
