As the Hospital Discharge Coordinator for SMI Patients at the Outpatient Clinic I am responsible for ensuring safe, effective, and timely transitions from inpatient psychiatric or medical care to community-based services for patients with severe mental illness. This role involves close collaboration and coordination between patients, families, and external support providers to develop individualized discharge plans that support recovery, reduce readmission risk, and promote continuity of care. In my role I facilitate referrals to community mental health providers, residential treatment programs, substance use services, housing supports, and all available community resources. In addition, I arranged placement of patients in facilities appropriate for level of care requirements. In this role I also authorized medically appropriate facility admissions and means of transportation.
As a leader in this role, I am responsible for coordinating and overseeing the care of our patients that suffer from severe mental illnesses, ensuring they receive appropriate mental health services. I complete intakes and assessments for new and existing patients, plan for discharges from hospitals, and act as an advocate to help clients achieve their goals and improve their overall well-being. I act as a support system for members facing serious mental crisis by coordinating care with insurance companies and hospitals, supervision officers, and families. In this role I am actively facilitating access to resources in the community, and promoting recovery and sobriety by using appropriate communication skills while working collaboratively with a team. I answer in-bound calls and emails daily and schedule appointments to ensure that our members receive the appropriate care they need and deserve.
I was successful in supervising, educating, and guiding probationers so they are able to complete their probation term. I managed over 150 clients on a monthly basis and became familiar with the different personalities that I faced. In this position, I acted as an advocate and the line of communication between clients and the Legal Courts. I was responsible for making sure that the community was a safer place in hopes that they do not re-offend or violate conditions of supervision. I was able to make a large impact on my clients lives often leading to successful discharge from probation.
