Provide professional, compassionate home and community based skilled nursing services based on provider orders and patient goals including, but not limited to wound care, blood draws, line care, tracheostomy care, diabetic monitoring, general health assessments, vital signs, ongoing medication management, care coordination, and education.
Provide compassionate, end-of-life care to terminally ill patients in their homes. Develop and implement care plans, manage symptoms and pain, provide emotional and spiritual support to patients and their families, and educate caregivers. Work closely with an interdisciplinary team to ensure holistic and dignified care. Respond to crisis situations, providing immediate care and support to patients and families. Conduct physical exams, assess for physiological changes associated with the end of life, and officially pronounce patient death, complete the necessary paperwork, and facilitate end of life arrangements.
Provide direct patient care, conduct patient assessments and on-going case management, utilize resources available to benefit the patient, perform community outreach and education, and advocate for appropriate patient care.
Provide direct inpatient care on the bone marrow transplant/ oncology unit. Assess patient needs, formulate a plan of care alongside health team, evaluate patient response and progress towards healthcare goals, and maintain standard of care within evidence based research and that are compliant with hospital policies and procedures.
