  • The Primary Care Management Module (PCMM) supports both primary care and non-primary care teams
  • This software allowed us to setup and outline a team, assigned positions to the team, assigned staff to the positions, assign patients to the team(s), assigned patients to practitioners, and reassigned patients from one team to another team
  • Manpower and Administrative: Duties include assisting Primary Care PACT teams and clinics with identifying and developing improvements and efficiency of manpower and methods that is presented to our leadership
  • Additionally, we (PCMM Coordinators) performed team reviews in the context of knowledge of CBOC clinic activities, skills, speed, volume, and quality requirements of the work processed
  • We performed management analyses, research projects and provided advisory services on technical compliance, workflow & process, and achievement of numerous and valued performance measures for Primary Care management and CBOC clinics
  • We maintained knowledge of regulations and guidelines working with National, VISN and local Facility Primary Care Coordinators when needed to possess the most up to date data
