Responsible for assisting in the day-to-day tasks to ensure that case management services are provided to youth in a timely manner.
Submit completed reunification packet with appropriate referral made by Case Manager for the timely release of youth to designated ORR representative.
Ensure the provision weekly telephonic contacts with family of origin primary caregiver and/or sponsor.
Facilitate attorney to youth contact as requested by youth.
Drive clients to facilitate program services, may include transportation to court appointments, attorney visits, other appointments as per contractual requirements as needed.
Ensure the provision of follow up service phone calls are conducted 30 days. after youth’s reunification and discharge from program.
Assist case management department with filing, closing files, and with the overall daily maintenance of records found in either the physical case file or on ETO.
Establishing and maintaining effective, working relationships with children, parents, staff, and professionals.
