Works as a LNA in the Clinic and in the Community Health.
In the Clinic; -
Functions works opening the clinic as daily routine, Checking and logging Temperature of the Vaccine Fridge, Laboratory fridge, Accucheck machine, Covid machine, making sure it is in working order.
-Triaging patient, and rooming in patient to see assigned provider, coordinates with the RN with assigned task.
-Works independently with or without supervision by the supervisor, and within the scope of practice.
-Perform Phlebotomy, EKG, Weighing Scale machine (Loss Weight Management), Blood pressure and Blood sugar check , A1C test, Urinalysis, Pregnancy, Drug test. Covid, Strep, Flu, RSV test. Eye screenings and Foot screenings. And reports to RN.
In Community Health:
-Home visits as assigned patient by a Case Manager RN.
-Perform health screenings, testing blood sugar levels, checking vital signs, home safety assessments, and foot screenings.
-Assist with medication management and self-administration.
-Evaluate and monitor the health status, and general well-being of Community members residing in the Community and visiting patient's who are in the private group homes, skilled nursing facilities, and Hospital visits with the RN.
-Assisting Patient in making appointments for referrals from there perspective providers, driving and accompanying to their appointments, advocating patients for their well being, keeping medication list updated during doctor's visit and making sure patients understands after each provider's visit.
-Making each patients a list of appointments, logging daily blood sugar, and a list of medications chart.
-Filling weekly medication box, calling pharmacy for refills. Assisting with transfer, repositioning, and assisting the family when needed. -
Perform basic wound care with the supervision of the wound care nurse and reports any changes.
-Perform Phlebotomy at home specially to homebound elderly and collect specimen for lab test in a timely manner.
-Assisting patient to senior center making sure patient received and did not miss any delivered lunch.
-Inspect homes (bed bugs infestation) and assist patient to improve safety, promote health, and reduce injury.
-Performed head lice check upon request of the family and the school.
-Any abnormalities must be reported to the Community Health Nurse.
-Assigned to oversee patient transportation (request and facilitates to the third party transportations company during weekends and Holidays to our dialysis patients ) and transport dialysis patients to their perspective assigned dialysis center and as needed.
-Participates and reports with other clinician such as, diabetic educator, dietitian, sports specialist activities and events.
