  • On behalf of the CEO of Haven Behavioral Hospital Phoenix, we are pleased to offer you the position of Registered Nurse. Subject to the conditions set forth below, this letter will serve to outline the basic terms of employment.
  • Start of Employment: You will start as a full-time employee effective 03/04/2024. This position is considered non-exempt status and is eligible for overtime pay for hours worked in excess of forty in a given work week. Your employment is contingent upon (i) successful completion of a pre-hire drug screen, (ii) satisfactory background checks (including, but not limited to, Federal and State background checks, reference checks, employment verification, and education verification).
  • Compensation: Effective 03/04/2024 you will be paid an hourly rate of $43.06. Employees are paid bi-weekly.
  • Schedule: Your regular schedule will include three 12-hour shifts per week. Your schedule may vary or a flexible schedule may be required to meet job responsibilities.
  • Orientation: New employee orientation consists of 5 days of classroom, web-based and hands-on training beginning 03/04/2024 at 9:00 a.m. Additional training will be arranged with you manager.
  • Benefits: As a full time employee you will be eligible to participate in Haven’s health & welfare benefits programs. This includes medical, dental, vision, life & disability benefits. Benefits are effective the first of the month following your date of hire. If you want health coverage or any other benefits, you MUST make your elections and enroll via Employee Self Service within 30 days from your date of hire. If you do not enroll within the 30 days, you will have to wait until Open Enrollment for the following Plan Year or experience a Qualifying Life Event during the year.
  • 401(k) Plan: All new employee will be automatically enrolled in the 401(k) plan at 2%. Upon eligibility, employees may receive a discretionary company match at the end of each plan year. You will have the option to opt out or change your election at any time. The Plan is effective the 1st of the month following 30 days of employment.
  • At-Will Employment: Haven Behavioral Healthcare is an at-will employer. Either you or the company are free to end the employment relationship at any time, with or without notice or cause. Nothing in this letter or Haven Behavioral Healthcare’s policies or procedures, either now or in the future, are intended to change the at-will nature of our relationship nor be construed as a contract.
  • After the acceptance of this offer you will receive a request to complete release forms for substance abuse test and background check through your ADP portal.
  • Prior to beginning employment on your first day, you will also need to provide the following documents:
  • Evidence of receiving the PPD test within the last 12 months prior to start date
  • Evidence of either receiving or waiving the Hepatitis B vaccination series
  • Evidence of flu vaccine if you have received it
  • All of the above vaccines and tests may be given at orientation
  • The following documents are to be brought with you on your first day of employment:
  • Copy of diploma/degree
  • Acceptable identification for employment eligibility verification (see I-9)
